The cleaver and axe signify the grave as being that of a flesher [butcher]. Above is a Green Man with tusks, holding a cornucopia in his jaws from which spill fruits and other greenery; below are the classic symbols of mortality: a skull and bones; all bordered by representations of vines and grapes.
