The last time I visited Carsington Reservoir was in early 1984. The dam embankment was almost complete but the pipeline to pump water from the River Derwent had been delayed following the fatal accident involving asphyxiation of four workmen in a storm drain. Shortly after this in June 1984 the dam failed with a 400m slip and large crack in the crest. The failure was investigated and the dam was redesigned as a much broader structure pushing back the opening until 1992.
